[…] Neuere Ergebnisse auf dem Sektor der Biomarkerforschung wurden auch in den Abschnitt zur klinisch apparenten Demenz integriert. Relevant ist hier, dass nicht unbedingt die Störung des Erinnerungsvermögens die Kardinalsymptomatik aus dem Kreis der kognitiven Dysfunktion bilden muss [3]. Allgemein betrachtet gehören Patienten, die bisher einer „wahrscheinlichen (‚probable’) AD“ zugeordnet wurden, auch nach neuer Klassifikation weiter in diese Gruppe, während teilweise Patienten, die bisher unter „mögliche (‚possible’) AD“ fielen, reevaluiert werden müssen. Dies ist auf zusätzliche Biomarkerbefunde und erweiterte klinische Befunde zurückzuführen, die nun zur Erhärtung der diagnostischen Sicherheit integriert wurden.
Wie auch bei den bisher genannten AD-Erkrankungsphasen unterscheiden die Autoren zwei Zielgruppen in der revidierten Fassung der diagnostischen Forschungskriterien: Einerseits richtet sich der überarbeitete Kriterienkatalog an Ärzte ohne Zugang zu ausführlichen neuropsychologischen Testungen, MRT, Nuklearmedizin oder einem neurochemischen Labor mit Expertise auf dem Gebiet der Liquoranalytik. Andererseits wird von klinischen Zentren, die diese zusätzliche biomarker-abhängige Validierung der Demenz-diagnose bereitstellen können, von den Autoren der überarbeiteten diagnostischen Forschungskriterien gefordert, zukünftig die Standardisierung und Qualitätskontrolle dieser biomarkergeleiteten Diagnostik weiter zu verbessern.

[…] More recent results from the area of biomarker research were also integrated in the section on clinically apparent dementia. Here, it is important to note that memory impairment does not necessarily have to be the core symptom among the group of cognitive deficits [3]. In general, patients who up until now have been categorized as having “probable AD” will remain in this group, even after renewed classification; whereas some of the patients who previously fell into the category of “possible AD” will require re-evaluation. This is a consequence of the additional biomarker evidence and expanded clinical findings that have now been integrated to improve diagnostic reliability.
As they do for the phases of AD disease mentioned above, the authors also identify two target groups in the revised version of the diagnostic research criteria: On the one hand, the re-worked catalogue of criteria targets those physicians, who have no access to extensive neuropsychological tests, MRT, nuclear medicine, or a neurochemistry laboratory with expertise in the area of cerebrospinal fluid analysis. On the other hand, where clinical centres are able to provide this additional biomarker-dependent validation of a dementia diagnosis, the authors of the revised diagnostic research criteria demand further improvement in the standardization and quality control of biomarker-led diagnosis in the future.
