My try:
The model will be implemented in an increasingly number of localities and it is expected to scale it up at national level by replicating the coordinated work under the government leadership, [thus] improving the sector mechanisms that will allow to deliver quality services to everybody around the country and that such services are sustained forever.

Me parece que la última parte "and that such services are sustained forever" no encaja bien.

Quizas "... and making such services to become sustainable forever/on a permanent basis/ on a regular basis"?
