Based on the measurement of 13.7cm, this is about pancreatic length. The craniocaudal dimension of the pancreas (with "cranial" and "caudal" referring to the axes of the patient's body) would be much smaller.
I have never seen any reference to a measurement "along the head-tail axis" of the pancreas, hence, I would suggest to keep it simple and just use "length". (On the other hand "eje CC" does not seem to be all that common in Spanish, so why not use an uncommon term like "head-tail axis" in English as well?)

: It is the spleen and not the pancreas, and yet I think your analysis is correct; long/longitudinal axis looks like the most natural option here. ¡Saludos!

Aunque la traducción natural de eje CC (céfalocaudal/cráneocaudal) es literal, al estar referido en este caso a una medida del bazo parece mucho más razonable aludir al eje longitudinal (longitudinal axis / long axis). El bazo no sitúa su eje largo (el que se utiliza como referencia) en una posición céfalocaudal sino en bipedestación; en decúbito, su eje mayor (longitudinal) se corresponde con el de la décima costilla.
Así, aunque no literal, diría que en este caso en particular eje CC puede interpretarse con seguridad como referido al eje longitudinal del bazo -tal y como comenta Anne en respuesta.
