Karina, you should probably qualify this as closed without grading due to Erroneous question. If you fill in "Reporte de evolucion clinico" as the Spanish term or phrase, then "Medical progress report" is an acceptable answer, but your question is erroneous since you actually ask for help with (some non-specified of) the abbreviations EVA, A, L, I, C, I, A.
