When deciding between easy and pro, err on the side of pro. Most questions will be pro.

If you fill in "Reporte de evolucion clinico" as the Spanish term or phrase, then "Medical progress report" is an acceptable answer, but your question is erroneous since you actually ask for help with (some non-specified of) the abbreviations EVA, A, L, I, C, I, A.
