Date of patient discharge or referral from nurse
En este caso, salvo que me esté perdiendo algo, ambas fechas estarían relacionadas (fecha de alta o derivación) ya que señalan el momento en que finaliza la atención de enfermería, bien por ser el paciente dado de alta o bien por haber sido derivado a otro profesional, servicio o centro sanitario. Creo que una forma de expresarlo en inglés sería como anotado en respuesta (date of discharge or referral from nurse). En este caso, el sentido no sería el de la fecha en que el paciente ha sido remitido a la enfermera sino la fecha en la que la enfermera remitió al paciente a otro profesional.
