We will actively SEEK customer input as we work towards a regular forecasting cycle by customer / part number for use in production planning as well as revenue planning.
Please can someone advise if the cycle is made by the manuf or by the customer or the cycle is preapred by the manuf and sent out to the customer to fill in, like a survey form.

I think the use of preposition "by" confused you regarding this sentence. It means "according to" in the sentence and then everything else is clear. There is no production/preparation of the cycle "by" customers. They will just provide inputs. The manufacturer will arrange the forecasting cycle according to such inputs.
