Back-Up

English Medical Medical: Instruments Ventillation
Context:
"The [device name] operates without Back-Up rate, whereas the other one can operate with or without Back-Up rate."

Elsewhere in the file, Back-up rate is defined as:
The rate corresponding to the respiratory cycle which will be provided when the device does not detect the patient respiratory cycle or when the patient spontaneous breathing is lower than the prescribed Back-Up rate.

Again, it's measured in bpm.
The preceding part speaks of inspiration and expiration, so I believe that both efforts might be related with both terms (Back) and (Up). (Both terms are in Capital letters throughout the document)

The Spanish version of the file translates the word as: Frecuencia respiratoria de reserva. Yet I can't understand what kind of "reservation" or storage is being talked about here, and how far it is related to the respiratory cycle context.

It's only a matter of whether this is a kind of "support" or a supplementary (ordinary) breath option when the patient's breath becomes lower than the expected rate?

A Bi-Level Positive Airway Pressure Device with a back-up rate (HCPCS K05333) also delivers adjustable, variable levels of positive air pressure to assist spontaneous respiratory efforts and supplement the volume of air a patient inspires. In addition to the automated delivery of bi-level air pressure, this device also has a time back-up feature to detect when a patient has stopped or delayed breathing. Whenever “sufficient spontaneous inspiratory efforts fail to occur” the back-up feature ensures that the appropriate amount of air is provided.

From your context and all of the examples you provided, it appears that they are using back-up in the same way you talk about a back up electrical generator which kicks in when the electrical system goes down.

Also the same as a computer back up.

You could think about it this way: a back up is that which gets you back up and running.

Bi-level pressure devices
VPAP or BiPAP (Variable/Bilevel Positive Airway Pressure) provides two levels of pressure: Inspiratory Positive Airway Pressure (IPAP) and a lower Expiratory Positive Airway Pressure (EPAP) for easier exhalation. (Some people use the term BPAP to parallel the terms APAP and CPAP.)

Modes
S (Spontaneous) - In spontaneous mode the device triggers IPAP when flow sensors detect spontaneous inspiratory effort and then cycles back to EPAP.

T (Timed) - In timed mode the IPAP/EPAP cycling is purely machine-triggered, at a set rate, typically expressed in breaths per minute (BPM).

S/T (Spontaneous/Timed) - Like spontaneous mode, the device triggers to IPAP on patient inspiratory effort. But in spontaneous/timed mode a "backup" rate is also set to ensure that patients still receives a minimum number of breaths per minute if they fail to breathe spontaneously.
