" En première instance " est l'expression utilisée par la pratique judiciaire pour désigner la partie de la phase procédurale qui, en cas d'appel, s'est déroulée avant la saisine de la Cour d'appel. Dans certains jugements on peut lire que la décision a été rendue " en première et dernière instance ", ce qui signifie qu'elle n'est pas susceptible d'appel.

HERE "instance" has the same meaning as in "en première instance" in the legal jargon.
toutes ... instances = can start a legal action (="en première instance") and if not satisfied keep appealing to higher and higher courts.
