medical discharge papers with a list of participating doctors.
there are five names altogether with their own related information, but these names appear in the following way: "XXX for last name, - then two Capital letters with a space but without periods (assuming it's a very strange way of writing intials) - and then this (вк).
Иванов И И (вк)
four times for four different people, and one time only the last name without intials or this "vk". The only thing I can think of of "врачебный консультант" but that's of course not correct.

some additional information: The name of the document is Эпикриз выписной (переводной) от такого-то числа.

am I to assume that this was translated in 2013, used supposedly and is now back translated? that would explain the very strange way of putting initials down after the last name. i still don't know what vk means.

Not really "translated". "Transfer" (to another medical establishment).
This form is filled in either at the patient's discharge/ exeat or in transfer, (underline as appropriate/strike out)
