I think this is the action of "antagonizing" what follows. If the patient is haemorrhaging, you have to counter the anticoagulants with antagonists of their action. The doctor is talking about the difficulty of doing just that (antagonist action on the anticoagulants that the patient has been taking, were given to him at some point, or occur naturally in his bloodstream). I don't think he's talking about the anticoagulants' antagonist action (on the patient's natural clotting factors).
