This is a sworn statement which has only been signed by the two parties (it hasn't been notarised). Therefore, when it refers to "testimonio", I don't believe it's referring to "testimonio notarial", ie "notarial certificate/record". I think it's just referring to the parties' testimony, ie their written statement.
I know that deeds are executed, but would it be correct to translate "formalizar" as "execute" in the context of "testimony"? If not, would it just be "formalise the following testimony"?

Funnily enough, West does give accused's 'formal statement' for testimonio indagatorio, but my hunch as an esrtwhile Commissioner for Oaths and notarially employed translator s something more legalistic.
