The entry for Full Address and Usual Place of R/o is the same, i.e. name of the village, the district etc.

Could R/o stand for something like Permanent Address?

The "of" seems superfluous though - Usual Place of Residence would be sufficient.
Acronym Finder gives R/O = Residence Of.
