It's talking about steps that can be taken during the design phase of buildings and facilities to minimize the need to climb on/into unsecure locations to operate or perform maintenance on equipment. Thinking about the layout of things so that components that things can be operated remotely, things that need frequent servicing can be easily accessed, and that there's a way to get at equipment once it is installed.
For example, my husband is a commercial HVAC mechanic, and one of the air handler units in the facilities he supervises is located at the top of a big gymnasium. If the architects had designed an access to this unit through the wall of the gym into the 3rd floor hallway on the other side, with secure platforms around the unit, it would be very simple to work on. Instead, they need to bring in a small "cherry picker" lift and involve a crew of 2 or 3 workers every time it needs maintenance.

I think Design out here means to plan carefully taking precautions, since one has to work at a high place (height) from preventing the risk of people getting injured from falling.
