Scusate la confusione. Avevo chiuso la domanda dall'inglese all'italiano perchè avevo sbagliato le combinazioni linguistiche ma vorrei che restasse dall'italiano all'inglese:

ha superato in data... con voti.... l'esame di specializzazione in CHIRURGIA PLASTICA E RICOSTRUTTIVA

Scusate, sarà banale ma devo scegliere tra postgraduate exam, final exam for specialising in... ecc., vorrei sapere se qualcuno conosce la formula che si usa nei cetificati di conseguimento di specializzazione. Grazie!

If you still can, say 'licensing examination'. It's less specific than my original answer, but still very pertinent to medicine. Personally, I'd include 'equivalent to USMLE Step 3' in parentheses, esp. if the translation goes oversees.
If I may add a comment, i'd leave out 'post graduate' in Cedric's suggestion, as that implies exams in other programs as well [e.g. MPH] plus specialization is a graduate program in teh US. Post-graduate is medical fellowship.

We need to adapt the translation for the target land, not worry about the Italian... So. if this translation 'goes to' US/Canada, board-certification for sure; for UK you need to ask someone else :)

Board-certification I believe Iirka is correct for the Canada-US exam. On the other hand, I don't believe that the same applies in Italy. Therefore to call this exam board certification in translation could be extending a common practice in N. America to an Italian practice that is not the same thing. On the other hand, it is up to you the translator to decide which term best fits your context and your client's needs.

That is what post graduate means - after having graduated with a degree.

Thank you Cedric, but can I be sure post graduate implies the idea of a post degree specialisation? The certificate relates to a young man who has taken his degree, then has attended a specialisation school and has passed the final exam!

board certification exam

In the US, after you've graduated from med school and done internship and residency [= specialization], you take a board-certification exam and become 'board-certified in...'

in your case:

'board-certification exam in plastic and reconstructive surgery'
