We disagree with the conclusion that antidepressants are not effective medications. While people respond differently to medications, there is a wealth of scientific evidence, including, but not limited to, academia and independent professional organizations, showing that antidepressants are effective, even in patients with low to moderate major depressive disorder (depression).

The recurring attention surrounding claims that antidepressants are not effective medications is a disservice to the millions of patients who suffer from depression and the dedicated health care professionals who treat them. It’s important that patients currently taking an antidepressant not stop their medication on their own. Rather, they should work with a health care provider to determine the best course of action for them.

Statements must be considered in the context of how placebos are used in clinical trials for depression. A placebo control in a clinical trial of depression is not the same as “no” treatment. In a clinical trial, patients who receive placebo also receive a diagnostic assessment by an experienced clinician and careful follow-up by trained staff, often for more than 10 hours during the course of a short-term study.

These actions are believed to have short-term therapeutic benefit, especially for patients with less severe illness. However, placebo responses tend to be short-lived, and numerous studies have shown that patients on placebo are more likely to relapse back into a depressive episode. [Reimherr F, Amsterdam JD, Quitkin FM, et al. Optimal length of continuation therapy in depression: a prospective assessment during long-term fluoxetine treatment. Am J Psychiatry. 1998;155(9):1247-1253. ]

Additionally, it is well known that drug-placebo differences are smaller with less severe disease, especially in short-term trials. Greater antidepressant-placebo differences are seen with longer-treatment trials.

In summary, there is a substantial body of scientific evidence that demonstrates the efficacy of antidepressants in treating depression. Virtually all major treatment guidelines worldwide support the role of antidepressants in the treatment of patients with depression.

Optimal length of continuation therapy in depression: a prospective assessment during long-term fluoxetine treatment. Am J Psychiatry. 1998;155(9):1247-1253. ]
