Source text - English According to the WHO, each year nearly 15 million people are having a stroke. And one third remains impaired after a stroke [Mac, p. 50]. In Germany, approx. 196,000 people are affected each year and the mortality rate within the first year after a stroke is at 30% [Did, p. 592]. In most cases, the affected cerebral areas are the ones containing the programmes to control the musculoskeletal system [Cor, p. 11]. Acting fast is important because the sooner a stroke is recognised and treated the better the secondary damages can be controlled. Thus, the German Neurological Society (DGN) calls for immediate orthotic fittings [Hes, p. 1150].
In addition, numerous clinical studies confirm the indispensability of orthoses for a gait rehabilitation after a stroke [Bow, p. 87ff.].
But there is still a lot of unused potential for the orthotic fitting of apoplexy patients as the technical preconditions meeting all the requirements for such an orthosis could not be realised so far.
Most of the orthotic concepts used until now for apoplexy patients have to be reconsidered due to the NEURO SWING system ankle joint that leads to new opportunities in orthotic management.
This Apoplexy Guide was created as a result to facilitate the communication between physicians, physiotherapists, orthopaedic technicians, orthopaedic shoemakers and biomechanics for the orthotic fitting of apoplexy patients. This guide provides adequate suggestions for an orthotic fitting based on practical experience and scientific knowledge. Last but not least the partner or attending persons of patients and, of course, the patients themselves are part of the communication towards a decision for an orthotic fitting.
The N.A.P. Gait Classification was developed in cooperation with qualified physiotherapists and orthopaedic technicians. This is an easy to apply gait classification for apoplexy patients and an important basis for the treatment concept at hand. In order to understand our concept, a basic knowledge of the physiological gait pattern is absolutely required. Therefore, the most important terms and gait phases are also explained in this guide.
Since all experts we asked had great interest, it was possible to bundle several research results and experiences. We would like to thank all the persons involved for the help received. And special thanks go to Renata Horst and Beate Hesse who, as a stroke patient, volunteered as model for our exemplary pictures.
We do not expect our Apoplexy Guide to be perfect. Rather, it should be the beginning of an innovative concept for the orthotic treatment of apoplexy and we rely on your comments and critique to continually improve its quality.
Translation - Czech Podle WHO ročně prodělá mrtvici téměř 15 milionů lidí. Jedna třetina z nich zůstává po mrtvici postižena [Mac, p. 50]. V Německu je tímto onemocněním postiženo cca 196 000 osob ročně a úmrtnost během prvního roku je 30 % [Did, p. 592]. Ve většině případů jsou poškozena mozková centra ovládající muskuloskeletální systém [Cor, p. 11]. Je důležité jednat rychle, protože čím dříve je mrtvice odhalena a léčena, tím lépe se dá zabránit sekundárnímu poškození. Proto Německá Neurologická Společnost (DGN) naléhavě volá po ortotickém vybavení [Hes, p. 1150]. Kromě toho početné klinické studie potvrzují nepostradatelnost ortéz pro rehabilitaci chůze po mrtvici [Bow, p. 87ff.].U pacientů s CMP stále existuje velký nevyužitý potenciál ortotických pomůcek vzhledem k dosavadní neschopnosti splnit u ortéz veškeré nezbytné technické předpoklady.Většina dosud používaných ortotických konceptů u pacientů s CMP musí být opětovně posouzena kvůli systému hlezenního kloubu NEURO SWING, který vede k novým možnostem ortotického řešení. Tato Příručka pro pacienty s CMP byla vytvořena jako prostředek k usnadnění komunikace mezi lékaři, fyzioterapeuty, ortopedickými techniky, výrobci ortopedických bot a biomechaniky pro ortotické řešení pacientů s CMP. Příručka poskytuje adekvátní návrhy ortotických pomůcek založené na praktických zkušenostech a vědeckých znalostech. V neposlední řadě jsou součástí komunikace vedoucí k rozhodnutí o ortotické léčbě partner, ošetřující osoba pacienta a samozřejmě pacient samotný.Ve spolupráci s kvalifikovanými fyzioterapeuty a ortopedickými techniky byla vytvořena Klasifikace chůze N.A.P. Je to snadno aplikovatelná klasifikace chůze pro pacienty po CMP a důležitý základ pro koncepci léčby. Pro pochopení našeho konceptu je naprosto nezbytná základní znalost fyziologického způsobu chůze. Proto jsou v této příručce vysvětleny také nejdůležitější termíny a fáze chůze.Vzhledem k tomu, že všichni oslovení odborníci projevili velký zájem, bylo možné dát dohromady hned několik výsledků výzkumů a zkušeností. Rádi bychom poděkovali všem zúčastněným osobám za poskytnutou pomoc. Zvláštní poděkování patří Renatě Horst a Beatě Hesse, které se, jakožto pacientky po CMP, dobrovolně přihlásily jako figurantky pro naše ilustrativní fotografie. Neočekáváme, že náš Průvodce pro pacienty po CMP bude perfektní. Spíše by měl být začátkem inovativní koncepce pro ortetickou léčbu mozkové mrtvice a spoléháme na Vaše komentáře a kritiku, abychom mohli neustále zlepšovat jeho kvalitu.
