Source text - Ukrainian Еплеренон вивчали у ході дослідження EPHESUS. Це було подвійне сліпе плацебо-контрольоване дослідження тривалістю 3 роки, в якому брало участь 6632 суб’єктів з гострим інфарктом міокарда, дисфункцією лівого шлуночка (оцінювалося за фракцією викиду лівого шлуночка ≤ 40 %) та клінічними ознаками серцевої недостатності. Через 3 14 днів після гострого інфаркту міокарда (медіана становила 7 днів) суб’єкти додатково до стандартного лікування отримували еплеренон або плацебо у початковій дозі 25 мг 1 раз на добу. У подальшому дозу поступово (впродовж 4 тижнів) підвищували до досягнення цільової дози 50 мг 1 раз на добу за умови, що рівень калію в сироватці крові був нижчим за 5 ммоль/л. Протягом дослідження суб’єкти отримували стандартне лікування, що включало ацетилсаліцилову кислоту (92 %), інгібітори АПФ (90 %), β-блокатори (83 %), нітрати (72 %), петльові діуретики (66 %) або інгібітори ГМГ-КоА-редуктази (60 %).
Первинними кінцевими точками у дослідженні EPHESUS були загальна летальність та комбінована кінцева точка (летальний наслідок або госпіталізація внаслідок порушень з боку серцево-судинної системи). 14,4 % суб’єктів із групи еплеренону та 16,7 % суб’єктів із групи плацебо померли (з будь-яких причин), а 26,7 % суб’єктів із групи еплеренону та 30 % суб’єктів із групи плацебо досягли відповідності критеріям комбінованої кінцевої точки (летальний наслідок або госпіталізація внаслідок порушень з боку серцево-судинної системи). Translation - English Eplerenone was evaluated during the EPHESUS study. This was a double-blind, placebo-controlled, 3-year study involving 6,632 subjects with acute MI, LV dysfunction (LV ejection fraction ≤ 40%), and clinical signs of heart failure. In 3-14 days after acute MI (the median was 7 days), subjects received eplerenone or placebo at an initial dose of 25 mg once a day in addition to standard treatment. Subsequently, the dose was gradually (within 4 weeks) increased to achieve a target dose of 50 mg once a day, provided that the potassium level in the blood serum was below 5 mmol/l. During the study, subjects received standard treatment including acetylsalicylic acid (92%), ACE inhibitors (90%), beta-blockers (83%), nitrates (72%), loop diuretics (66%) or HMG-CoA reductase inhibitors (60%).
The primary endpoints in EPHESUS were total mortality and combined endpoint (death from/hospitalization for cardiovascular causes). 14.4% of patients in the eplerenone group and 16.7% of patients in the placebo group died (from any cause) and 26.7% of patients in the eplerenone group and 30% of patients in the placebo group achieved compliance with the combined endpoint criteria (death from/hospitalization for cardiovascular causes). Thus, in the EPHESUS trial, eplerenone reduced the risk of all-cause mortality by 15% (relative risk 0.85, 95% CI 0.75-0.96, p = 0.008) compared with placebo, mainly due to a reduction in mortality from cardiovascular disorders. Also eplerenone redused the risk of death or hospitalization due to cardiovascular causes by 13% (relative risk 0.87, 95% CI 0.79-0.95, p = 0.002).

Preparation:
Note: Do not use a tourniquet concurrently with this device.
1. Ensure all components of the device are at operating temperature before proceeding.
2. Batteries must be installed into the device prior to use (reference the Maintenance and Storage section for installation/replacement).
3. Prepare Electrodes:
a. Per the electrode images, place the electrodes on the selected arm for venous access/IV placement (white electrode patch on the palmar surface of the hand and the blue electrode patch on the bicep of the arm) (See Figure 2).
Note: electrodes can be placed in any orientation as long as the location is maintained.

b. Connect the electrodes to the stimulator using the electrode cable
Note: In order to fully connect, the ridge on the plug must be fully inserted into the connector (See Figure 3).
Treatment:
1. Turn on the unit by rotating the knob CW to setting “1” and observe the physical response. If muscle fasciculation is not observed slowly increase the intensity until a response is observed.
Note: The patient will experience tingling, but the sensation should not be painful. If the sensation is not tolerable, lower the intensity to a tolerable level that still produces muscle fasciculation.
2. Continue the stimulation for at least 2 minutes, but no more than 10 minutes.
Note: The device will turn off on its own if the treatment time is exceeded.
3. Discontinue stimulation when the target vein has become visible or palpable.
4. Turn off the control unit by rotating the knob CCW until the power on/off light turns off.
5. Remove electrodes from the patient.
Venous Access/Venipuncture Procedure:

Source text - English To make your own protein packed flatbreads simply add 3 large scoops (100g) of powder to 120-150ml of warm water and 2 tsp of olive oil. Mix the powder and then form a soft dough, knead for 5 minutes and then set aside. Separate into 4 balls and roll out to strips. Griddle on a high heat for 2 minutes or bake for 10-12 minutes. Full directions and recipe ideas can be found on the Myprotein website.
