född bakom en mjölkpall
This a really old expression meaning: I´m not stupid or I wasn´t born yesterday. (of course, I haven't seen the whole sentence)

helt bakom
When someone says Were you raised in a barn?, it means that you are completly stupid. And you answer I am not raised in a barn.

The phrase "were you raised in a barn?" is generally used to indicate that a person is failing to engage in "human" behavior as opposed to living with animals in a barn where, for example, the animals do not care if you leave the door open, leave clothes and other objects strewn about, etc.
