My point of view, even though my answer might not be the best one, is that the sentence wants to find out 3 key factors i.e. a) the asker's question (what kind of causes), b) la frequenza degli avvenimenti/accadimenti, c) gli effetti conseguenti. Otherwise the Italian would have been something like: Di seguito si riporta la tabella riepilogativa degli scenari incidentali analizzati, con cause, frequenze ed effetti.

Il problema é come Nella mia opinione, qui sono possibili due letture: semplicemente il fattore scatenante, cioè "the cause", e buna notte; oppure la concausa (vedi la mia proposta), cioè qualcosa che facilita l'incidente, lo rende più probabile. Infatti, tali concause possono anche essere considerate come i fattori attenuanti, almeno per quanto riguarda la risponsabilità penale. A questo punto the qualifier "portarice" diventa cruciale e non può essere trascurato.

agree Linda Thody : absolutely! Yes Colin has a point, but I think "promotrici" should be translated somehow. Otherwise why put it in Italian in the first place? It is/was not strictly necessary. I agree with tradugrace here; it appears to be underlining the causes
