scope of work (list)
Where I live, our management office wants to see a scope of work on the contractor's own letterhead, before somebody does any work in the building. They also want to see liability insurance and workers' comp, of course, but that scope of work must include all the things Marc mentioned above.

A scope of work is detailing tasks (http://www.businessdictionary.com/definition/scope-of-work.h... ), but I am not so sure about the cost of materials.
Especially because a "scope of work" includes pricing, but this one does not, as it seems.
In tendering/bidding (if you disregard the "cost" part for a moment), you'd call that a Leistungsverzeichnis or bill of quantities:
