I never like the word "gestionar" its on par with "tramitar" for its ambiguity.
Anyway, as patients are people and not things, I'd go with seen rather than managed or handled or the like

I don't think "where patients are managed" is appropriate in this context. You could just say: "A value-added program is one in which patients are offered a range of services and benefits".
