A police report from Andorra. They had to ask the hospital to take a blood test from the injured party because he could not take a breathalyser test.

I'm not sure of the correct term in English for this.

Many thanks in advance!

notice requiring compliance

If the question is expanded with the analysis, then there's no need to pin down the requisition part: notice requiring the conducting of an analysis.

I don't believe there's a formal procedure for such a police request in the UK.
Example sentence:

Police and Criminal Evidence Act 1984 (PACE) Police and Forensic physicians may be asked by the police to take a blood sample www.bma.org.uk/images/healthdetainees0209_tcm41-183353.pdf

formal instruction

I'm not sure that there is an English equivalent for this. The IEC gives "Instigació verbal o escrita, feta mitjançant una autoritat pública o una persona, perquè executi o s'abstingui d'executar un acte determinat." for 'requeriment'. The situation seems to be that a certain formality is required in Andorra (hence the 'diligència') for this type of obligatory medical intervention.

formal request

I have no problems with Nigel's answer but you might want to consider this as an alternative.
Peer comment(s):

neutral Nigel Wheatley : My only problem with this suggestion is that a "request" can be refused, whereas the sense of 'requeriment' implies obligation to conform. // was meant to be 'neutral, not 'disagree'
